Mail Requirements:

Step 1.

Proof-of-purchase tab from your Disney•Pixar UP Blu-rayTM or DVD. Proof-of-purchase tab is located on the back just above the UPC code.

Step 2.

Cash register receipt from your UP Blu-ray or DVD purchase imprinted with store name and dated between November 10, 2009 and January 15, 2010 with purchase price circled (photocopies acceptable).

Step 3.

Cash register receipt from pasta brand purchase imprinted with store name and dated between November 10, 2009 and January 15, 2010 with purchase price circled (photocopies acceptable.)

Step 4.

Completed certificate from website or information can be included on an index card.

You will receive a $4 rebate by mail. Please allow 6-8 weeks for check to arrive. Rebate is not payable at the retail store.
